We are delighted to announce that Senior Care Assistant Tracy
Wallace has successfully completed the ‘Six Steps to Success’ training
programme with the Wirral NHS Foundation Trust, which has enhanced her own
understanding and the quality of end of life care provided throughout Summer
Fields Care Home. Tracy has worked tirelessly to become our home’s End of Life
Champion, and we couldn’t be prouder of her determination and effort. End of life
care is an essential part of what we do, and the programme embeds a philosophy
of palliative care while supporting those working with residents from all walks
of life.
Tracy’s persistent efforts have helped to introduce
appropriate end of life care documents to those that need them, while ensuring
that many residents have Emergency Health Care Plans. In addition, many of her
colleagues are now more confident in the delivery of end of life care, and are
empowered to speak up when they are not sure. The excellent standard of care
provided by Summer Fields Care Home can reach new heights, and give residents
and their families even more reason to feel reassured.
The Six Steps to
Success in End of Life Training
The Six Steps to Success for End of Life Training Programme
consists of six steps, which have been carefully created for the delivery of
rounded, thorough care for those approaching end of life. The programme
includes discussions with staff members, loved ones and care professionals at
every stage, as well as regular assessments to ensure that plans remain within
residents’ best interests.

The training programme empowers care assistants to identify
those nearing end of life, and carry out holistic, medical, spiritual and
mental capacity assessments to create an EHCP that is in the resident’s best
interest. Tracy has been encouraged to ask questions and consider residents’
wishes; what music do they like? Is there a particular scent, sound or
belonging that they’d find most comforting? End of life care extends beyond
medical observations, and encompasses faith and personal wishes, too.
Above all Tracy is encouraged to be proactive in all that
she does; sharing information and supporting others is vital for an End of Life
Champion. She must be confident to have difficult conversations with family
members and friends, and recognise when a care plan must change. We are
confident that Tracy has the knowledge and self-assurance to succeed in her new
role as End of Life Champion, and will continue to improve the quality of care
on offer at Summer Fields Care Home.
Manager Kelly Crabtree says, “We are extremely proud of
Tracy, and everything she has achieved during this course. She has a real
enthusiasm and passion for her job, which is apparent in everything that she
does. Her work ethic is infectious, and we are confident that she will support
the rest of our team as we prepare to offer heightened end of life care to
those in need and their families.”
